Job Description

Job Summary

Responsibilities may include support in the areas of scheduling, intake and referral or insurance case management as needed to facilitate the operations of Evergreen Home Care Services (EHCS) which includes Home Health, Hospice and Palliative Care.

Accountabilities/Competencies

1. Proficiency in reading, writing, comprehending and speaking English as required for business necessity.

2. Accountability for HIPAA privacy and security standards within the department or assigned nursing unit.

3. Must possess basic skills to use Microsoft Office (VISIO & Power Point), specialized software and electronic office equipment.

4. Parts of the work involve multiple steps and are quite complex, requiring an attention to detail and creative problem-solving skills. Accuracy is essential, as the tasks of the CSS impact the delivery and documentation of patient/client care.

5. Clinical Team Support:

•Schedules patient/client visits for all clinical disciplines.

•Coordinates assignment of individual cases to maximize utilization of resources.

•Enters clinical data into the paper or electronic medical record.

•Produces and distributes reports related to clinical data.

•Works with other EvergreenHealth departments to obtain information regarding patient/client scheduling.

•Responds to all internal and external customer inquiries.

•Provides clerical support to clinical staff members as needed.

6. Intake and Referral Support:

•Pre-registers patients/clients for visits, gathers and organizes demographic and referral information and completes all insurance verifications.

•Contacts the insurance carrier and obtains initial insurance authorization for Home Health, Hospice and

Behavioral Health patients/clients.

•Routes various financial letters to patients/clients or other responsible party based on quoted benefits.

•Updates patient/client information in the electronic medical record as needed.

•Updates physician information in the Physician Information data base as needed.

•Notifies referral sources of service availability to promote increased referrals.

•Maintains referral availability logs.

•Generates various intake and referral reports on a regular basis.

7. Business Office Support:

•Completes electronic/manual billing, cash posting, account reconciliation and collections for Medicare, Medicaid, Third Party Insurers and Self Pay.

•Prepares and generates analytical reports monthly that summarizes status of accounts.

•Assists patients/clients with billing questions and contacts patients/clients regarding individual accounts.

•Processes purchase order requisitions.

•Process invoices related to the patients hospice diagnosis and covered under the Hospice Plan of Care.

•Manages the Hospice Medicaid Nursing Home "pass through" program and coordinates with Business Operations Manager and other Evergreen Healthcare departments to ensure proper billing and reimbursement for Medicaid claims.

Minimum Qualifications


License, Certification, Education or Experience: 


REQUIRED for the position:

High School graduate or equivalent

Proficient in Medical Terminology

Knowledge of quality improvement initiatives.

Knowledge and familiarity of contracts, including terms and rates.

DESIRED for the position:

1 year of medical field experience
