Licensed Practical Nurse - Hospice
*****Up to $2100.00 sign-on bonus paid out over one year for those new to EvergreenHealth*****
Responsible for providing nursing services to patients in the home under the direct supervision of a registered nurse following a plan of treatment as ordered by the physician.
•To be successful, clinicians must be organized, flexible, creative, and have critical thinking skills. Must be able adapt to a wide variety of home environments and fluctuating schedules, and successfully master technology in the workplace. Clinicians must be excellent communicators, communicating effectively to physicians, insurance case managers, and other team members over the phone, in writing, and in person.
•Must utilize Electronic medical Records (EMR) and other related applications to deliver and track patient care.
•Responsible for accessing high speed internet at least twice daily to send and receive information/records.
•Must utilize a laptop and other job related technology in non-traditional settings; car, patient’s homes, public buildings. Must be able to adapt to using a keyboard and electronic equipment in non-traditional settings.
License, Certification, Education or Experience:
REQUIRED for the position:
•Currently licensed as a LPN in the State of Washington
•Current Washington State Driver’s license and proof of insurability
•Current Healthcare Provider BLS certification by date of hire
•Demonstrates ability to assess, plan, implement, and evaluate patient care.
•Demonstrates ability to communicate clinical / educational knowledge to others (i.e., students, staff, patients and families, caregivers).
•Demonstrates skills to complete the nursing competency assessment checklist.
•Demonstrates knowledge of new trends and techniques in nursing.
•Demonstrates oral, written, visual, and auditory skills in order to communicate, deliver safe patient care, and document patient care.
•Demonstrates the ability to navigate to patient homes, and drive in varied situations and inclement weather.
•Demonstrate efficient time management skills to independently manage assignment, i.e. to schedule visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns.
•Demonstrates an ability to utilize various teaching methods and to adapt these to patient and family education level and learning style.
•Demonstrate an understanding of the healthcare financial environment, reimbursement, Conditions of Participation and Joint Commission standards.
•Demonstrates knowledge and skills to care for specific age groups under his/her care (i.e., newborn, pediatric, adult, geriatric).
•Demonstrates the ability to perform an assessment process that includes age specific growth and development needs.
•Demonstrates accountability for professional nursing practice, commitment to patient care and commitment to uphold the Patient Bill of Rights.
•Must have interpersonal skills necessary to deal effectively with patients/families from a variety of backgrounds and with varied emotional and medical needs.
•Demonstrates ability to function as part of an interdisciplinary team.
DESIRED for the position:
•3 years of recent applicable clinical experience as a Licensed Practical Nurse, preferably in a medical/surgical unit, home health or Hospice home care setting
